Background

Protein Structure
IRS proteins share common structural features that enable their function:
- PH (Pleckstrin Homology) Domain: Located at the N-terminus, the PH domain binds to phosphoinositides in the plasma membrane, facilitating the localization of IRS proteins to the cell membrane.
- PTB (Phosphotyrosine Binding) Domain: Adjacent to the PH domain, the PTB domain binds to specific phosphotyrosine residues on the insulin receptor or other docking proteins.
- Multiple Tyrosine Phosphorylation Sites: Scattered throughout the IRS protein are several tyrosine residues that, when phosphorylated, serve as binding sites for SH2 domain-containing proteins such as PI3K and Grb2.
- Serine/Threonine Phosphorylation Sites: These sites can be phosphorylated by various kinases, leading to either the enhancement or inhibition of IRS signaling.
Classification and Subtypes
The IRS protein family includes several members, each with distinct roles and expression patterns:
- IRS1: Widely expressed and primarily involved in metabolic actions of insulin in muscle and adipose tissues.
- IRS2: Plays a critical role in insulin signaling in the liver and pancreatic β-cells, also implicated in brain functions.
- IRS3 and IRS4: Less is known about these members; they have more restricted tissue distribution and specific roles in certain tissues.

- Obesity: Altered IRS signaling pathways contribute to the development and maintenance of obesity by affecting energy balance and adipogenesis.
- Cancer: Aberrant IRS signaling can promote tumorigenesis through the activation of growth and survival pathways, particularly in cancers such as breast and prostate cancer.
