The pivotal role of IL-15 in immune system regulation, particularly its involvement in the homeostasis of NK and memory CD8+ T cells, has made it an attractive target for therapeutic manipulation, particularly in cancer immunotherapy, where enhancing immune responses can lead to improved tumor control.


Protein Structure

IL-15 is a small cytokine with a molecular weight of approximately 14-15 kDa, consisting of 114 amino acids. It belongs to the four-helix bundle family of cytokines, a structural group characterized by their four alpha-helical regions.

  • Four-Helix Bundle: Like many other cytokines, IL-15 adopts a four-helix bundle configuration. This structure is crucial for its biological activity and interaction with its specific receptor subunits. IL-15 shares about 45% amino acid sequence homology with IL-2, particularly in regions that mediate receptor binding. Classification and Subtypes

These effects are exerted through trans-presentation, wherein IL-15 binds to IL-15Rα on the surface of presenting cells (such as dendritic cells or macrophages) and is delivered to neighboring cells expressing IL-2/IL-15Rβγ.

  • NK Cell Development and Survival: IL-15 is essential for the development, activation, and maintenance of natural killer (NK) cells, a key component of the innate immune system.   • Immunosurveillance: By maintaining the functional capacity of cytotoxic lymphocytes, IL-15 plays a key role in immunosurveillance, helping the immune system recognize and eliminate abnormal cells, including tumor cells and virus-infected cells.
  • Inflammatory Response: IL-15 is produced during inflammation and contributes to the amplification of immune responses. It can induce the production of other inflammatory cytokines, such as TNF-α and IL-1β, promoting a robust immune response.
Biological Significance:

The ability of IL-15 to enhance cytotoxic immune responses makes it a crucial component of the body's defenses against intracellular pathogens and cancer. Its role in maintaining memory CD8+ T cells ensures that the immune system can mount rapid and effective responses to previously encountered pathogens, providing long-lasting immunity.

  • Inflammatory Bowel Disease (IBD): IL-15 contributes to the pathology of IBD by driving the activation of intestinal immune cells and promoting chronic inflammation.
  • Celiac Disease: In celiac disease, IL-15 overexpression in the intestinal epithelium leads to the activation of intraepithelial lymphocytes, causing tissue damage and the destruction of intestinal villi in response to gluten ingestion.

Clinical trials are investigating recombinant IL-15 and IL-15 superagonists as treatments for various cancers, including melanoma, renal cell carcinoma, and lymphomas. The goal is to boost the body’s natural immune response to eliminate cancer cells.
  • Immunotherapy Combinations: IL-15 is also being tested in combination with other immunotherapeutic agents, such as checkpoint inhibitors (e.g., anti-PD-1 or anti-CTLA-4 antibodies), to enhance their effectiveness by stimulating a stronger anti-tumor immune response.
  • Treatment of Viral Infections: IL-15’s role in enhancing the function of NK and CD8+ T cells makes it a candidate for the treatment of chronic viral infections, such as HIV and hepatitis C. IL-15-based therapies could help boost the immune system’s ability to control or eliminate persistent viral infections.
