Background

CR2, also known as complement receptor type 2 (CR2) or CD21, is a crucial protein involved in the immune response, particularly in the regulation and activation of B cells and the complement system. It serves as a receptor for fragments of complement component C3, specifically C3d, C3dg, and iC3b, which are breakdown products of C3b, an integral part of the complement system. By binding these complement fragments, CR2 plays a key role in amplifying the immune response and facilitating interactions between the innate and adaptive arms of the immune system.

CR2 is predominantly expressed on the surface of B lymphocytes, follicular dendritic cells, and some epithelial cells. Its most well-known function is to enhance B-cell activation by linking the complement system to the B-cell receptor (BCR) signaling pathway. This enhances B-cell sensitivity to antigens, especially during the formation of immunological memory. Additionally, CR2 acts as a receptor for the Epstein-Barr virus (EBV), allowing the virus to infect B cells and establishing a latent infection.

Given its involvement in both immune regulation and pathogen recognition, CR2 is a protein of significant interest in immunology and clinical medicine, with implications in autoimmune diseases, immune deficiencies, and viral infections.


Protein Structure

CR2 (CD21) is a 145 kDa glycoprotein that belongs to the complement control protein (CCP) family, also known as sushi domain-containing proteins. The structure of CR2 is composed of the following key features:

Extracellular Domain:

  • The extracellular portion of CR2 contains 15–16 short consensus repeats (SCRs), also known as sushi domains. These SCRs are approximately 60 amino acids long and adopt a beta-sheet fold that allows CR2 to bind complement fragments such as C3d, C3dg, and iC3b.
  • The first two SCR domains (SCR 1 and SCR 2) are primarily responsible for binding the C3d fragment, which is critical for CR2’s function in amplifying the BCR signaling pathway.
  • The remaining SCRs participate in other interactions, including binding to ligands like CD23, as well as interactions with other components of the immune system.

Transmembrane Domain:

  • CR2 spans the membrane with a single hydrophobic transmembrane region that anchors the protein into the B-cell membrane. This domain allows CR2 to interact with intracellular signaling molecules once activated by its ligands.

Cytoplasmic Domain:

  • The cytoplasmic tail of CR2 is relatively short and does not contain intrinsic enzymatic activity. However, it interacts with various intracellular signaling molecules, facilitating downstream signaling events after ligand binding. These interactions help to coordinate the complement-BCR signaling cascade, which is essential for B-cell activation and the immune response.

Oligomerization:

  • CR2 can form homodimers or potentially larger complexes with other membrane proteins, which may further enhance its role in signal transduction and immune complex formation.


Classification and Subtypes

CR2 is a member of the complement receptor family, specifically binding fragments of complement component C3. While CR2 is the predominant receptor for C3d, it has close relationships with other complement receptors, such as CR1 (CD35), which binds C3b and C4b. However, CR1 and CR2 differ in their ligand specificity and function within the immune system.

Although CR2 is commonly referred to in the context of B cells, alternative isoforms of CR2 may exist due to alternative splicing, resulting in proteins with minor structural differences. These isoforms might have varying expression patterns or ligand-binding properties, although the predominant and functionally important form is expressed on B cells.


Function and Biological Significance

The primary function of CR2 is to enhance B-cell activation and the immune response by serving as a coreceptor for the B-cell receptor (BCR). By binding C3d-coated antigens, CR2 brings the antigen and BCR into close proximity, thus lowering the activation threshold of B cells and allowing more efficient immune responses. The following summarizes the major functions of CR2:

Enhancement of B-Cell Activation:

  • CR2 significantly enhances the activation of B cells by binding to antigens opsonized with C3d. When C3d-coated antigens engage both CR2 and the BCR, a co-stimulatory signal is delivered to the B cell. This co-ligation amplifies the signaling cascade initiated by the BCR, resulting in more robust B-cell activation, proliferation, and antibody production.
  • This mechanism plays an essential role in the formation of memory B cells and the generation of high-affinity antibodies during an immune response.

Role in Complement System Regulation:

  • CR2 acts as a bridge between the innate and adaptive immune systems. By binding to complement-opsonized antigens, CR2 helps to clear immune complexes from the bloodstream and tissues, thereby preventing immune complex-mediated inflammation and tissue damage.
  • It also participates in the localization of antigens to follicular dendritic cells (FDCs) in lymphoid follicles, which is important for antigen presentation and the generation of long-term immune memory.

Interaction with Epstein-Barr Virus (EBV):

  • CR2 is the receptor that facilitates the entry of Epstein-Barr virus (EBV) into B cells. EBV binds to CR2 on the surface of B cells and uses this interaction to infect the cells, leading to the establishment of a latent infection that can persist for the lifetime of the host.
  • This interaction is crucial in the pathogenesis of diseases such as infectious mononucleosis and certain types of B-cell lymphomas, especially in immunocompromised individuals.

Regulation of Immune Tolerance and Autoimmunity:

  • CR2 has been implicated in the regulation of immune tolerance. Dysregulation of CR2 expression or function can contribute to the development of autoimmune diseases, where the immune system erroneously targets self-antigens.
  • In certain autoimmune conditions, CR2 levels may be altered, leading to impaired B-cell regulation and increased production of autoantibodies.


Clinical Issues

Given its central role in B-cell activation, complement regulation, and interaction with pathogens, CR2 is involved in several clinical conditions:

B-Cell Malignancies:

  • CR2 is expressed on B cells and certain B-cell malignancies, such as chronic lymphocytic leukemia (CLL) and some types of non-Hodgkin lymphoma (NHL). The expression of CR2 on malignant B cells has made it a target for therapeutic strategies aimed at disrupting B-cell signaling pathways.

Autoimmune Diseases:

  • Abnormalities in CR2 expression or function have been linked to autoimmune diseases such as systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA). In these diseases, defective CR2-mediated regulation of B-cell activation can lead to the production of autoantibodies and chronic inflammation.
  • Some studies suggest that targeting CR2 could be a potential therapeutic strategy to dampen B-cell hyperactivity and reduce the production of pathogenic autoantibodies.

Epstein-Barr Virus (EBV) Infection:

  • CR2 is the primary receptor for EBV, which causes infectious mononucleosis and is associated with various malignancies, including Burkitt’s lymphoma and nasopharyngeal carcinoma. Targeting CR2-EBV interactions could provide therapeutic benefits in controlling EBV infection and preventing its associated malignancies.

Immunodeficiencies:

  • Genetic mutations affecting CR2 can lead to immunodeficiencies, where individuals have an impaired ability to mount effective immune responses. These deficiencies can manifest as increased susceptibility to infections, impaired antibody responses, and difficulty in clearing immune complexes from circulation.
