Background:

Advanced glycation end products (AGEs) are a complex group of compounds that form through non-enzymatic reactions between reducing sugars and proteins, lipids, or nucleic acids. These reactions, known as glycation or Maillard reactions, occur spontaneously and accelerate under conditions of hyperglycemia (elevated blood sugar levels) and oxidative stress. AGEs accumulate in various tissues over time, particularly in long-lived proteins like collagen, and their formation is associated with aging and several chronic diseases, including diabetes, cardiovascular disease, Alzheimer's disease, and kidney disease. AGEs are formed through a series of chemical reactions between reducing sugars and proteins, lipids, or nucleic acids. These reactions result in the irreversible modification of biomolecules, leading to changes in their structure and function. AGEs accumulate in tissues over time, particularly in long-lived proteins such as collagen. This accumulation contributes to the pathogenesis of various age-related diseases. AGEs can cross-link with proteins, leading to the formation of advanced glycation end product-crosslinks (AGE-crosslinks). This cross-linking alters the structure and function of proteins and contributes to tissue stiffness and dysfunction. AGEs can also bind to cell surface receptors known as receptor for advanced glycation end products (RAGEs), activating signaling pathways that promote inflammation, oxidative stress, and tissue damage. AGEs can induce oxidative stress by generating reactive oxygen species (ROS) and impairing antioxidant defenses, leading to cellular damage and dysfunction. AGE accumulation and RAGE activation have been implicated in the pathogenesis of various diseases, including diabetes complications (e.g., nephropathy, retinopathy), cardiovascular disease (e.g., atherosclerosis), neurodegenerative diseases (e.g., Alzheimer's disease), and chronic kidney disease.

AGEs are diverse molecules resulting from the reaction between reducing sugars and the amino groups of proteins, lipids, or nucleic acids. Common structural features include:

  • Cross-Linking: AGEs often form cross-links between molecules, altering their structure and function.
  • Fluorescent Properties: Many AGEs exhibit fluorescence due to their complex molecular structures.
  • Heterogeneous Composition: AGEs comprise a wide variety of molecular species, such as carboxymethyl-lysine (CML), pentosidine, and pyrraline.


Classification and Subtypes:

AGEs can be classified based on their origin and chemical properties:

  • Endogenous AGEs: Formed naturally within the body through metabolic processes.
  • Exogenous AGEs: Derived from external sources such as diet (e.g., cooked foods) and tobacco smoke.
  • Types of AGEs: Specific types include CML, methylglyoxal-derived hydroimidazolones (MG-H1), and glucosepane.
