Background:

The ALK (anaplastic lymphoma kinase) gene encodes a receptor tyrosine kinase, which is a type of cell surface receptor that plays a role in signaling pathways involved in cell growth, proliferation, and differentiation. The ALK protein belongs to the insulin receptor superfamily and is normally expressed in specific tissues during embryonic development, where it helps regulate normal cell growth and development. While ALK expression is typically limited to certain tissues during development, aberrant activation of the ALK gene can occur in various cancers due to chromosomal rearrangements, gene amplification, or activating mutations. These alterations lead to the overexpression or constitutive activation of the ALK protein, contributing to oncogenesis (the formation of cancer).The most well-known oncogenic alteration involving the ALK gene is the fusion of the ALK gene with another gene, resulting in a chimeric fusion protein with constitutive kinase activity. This fusion protein can drive cellular transformation and tumorigenesis. One of the most common fusion partners of ALK is the EML4 (echinoderm microtubule-associated protein-like 4) gene, leading to the formation of the EML4-ALK fusion protein. This fusion is found in a subset of non-small cell lung cancers (NSCLC) and is associated with sensitivity to targeted therapies known as ALK inhibitors.ALK rearrangements have been identified in other cancer types, including anaplastic large cell lymphoma (ALCL), inflammatory myofibroblastic tumors (IMTs), and neuroblastoma. Detection of ALK alterations in these cancers can guide treatment decisions and facilitate the use of targeted therapies aimed at inhibiting ALK signaling.

  • Domains: The ALK protein consists of an extracellular ligand-binding domain, a single transmembrane helix, and an intracellular tyrosine kinase domain.
  • Ligand Binding: The extracellular domain binds to specific ligands, which triggers receptor dimerization and activation of its kinase domain.
  • Kinase Domain: The intracellular domain phosphorylates tyrosine residues on itself and on downstream signaling proteins, activating various signaling pathways.


Classification and Subtypes:

  • Isoforms: Alternative splicing of the ALK gene results in multiple transcript variants encoding different isoforms.
  • Homologs: ALK has homologs in various species, indicating its conserved role in development.


Function and Biological Significance:

  • Signal Transduction: ALK activates several signaling pathways, including the PI3K/AKT, RAS/MAPK, and JAK/STAT pathways, which are involved in cell proliferation, survival, and differentiation.
  • Development: ALK is crucial for the normal development of the nervous system. It regulates the survival of specific neuron populations and is involved in synaptic plasticity.
  • Cancer: Dysregulation of ALK, due to mutations, amplifications, or translocations, can lead to uncontrolled cell growth and cancer. Notably, ALK rearrangements are found in several cancers, including non-small cell lung cancer (NSCLC), neuroblastoma, and ALCL.

  • Cancer Therapy: ALK inhibitors, such as crizotinib, ceritinib, and alectinib, are used to treat cancers with ALK alterations. These drugs block the kinase activity of ALK, inhibiting tumor growth.
  • Resistance: Tumors can develop resistance to ALK inhibitors through secondary mutations in the ALK gene or activation of alternative signaling pathways. Ongoing research aims to overcome this resistance with next-generation inhibitors.
