SERPINA4 inhibits human amidolytic and kininogenase activities of tissue kallikrein. The inhibition is achieved by formation of an equimolar, heat and SDS-stable complex between the inhibitor and the enzyme, and generation of a small C-terminal fragment of the inhibitor due to cleavage at the reactive site by tissue kallikrein.

Serpins are a broadly distributed protease inhibitors that use the conformational change as a mechanism of action. They are central in controlling many important proteolytic cascades. Structuraly Serpinsare three β-sheets (A, B and C), 8-9 α-helices (termed hA-hI) and the reactive center known as reactive center loop (RCL). Kallistatin (SERPINA4) is a serine proteinase inhibitor and a heparin-binding protein. It is primarily found in vascular smooth muscle cells and endothelial cells within blood vessels, indicating its potential role in regulating vascular function.
