NPFFR2, also known as GPR74, is a G protein-coupled receptor that binds neuropeptide FF (NPFF) and related RFamide peptides. NPFFR2 is expressed in the central nervous system, particularly in the spinal cord, hypothalamus, and brainstem, as well as in peripheral tissues, where it modulates pain signaling, opioid responses, and energy homeostasis. NPFFR2 activation predominantly couples to Gi/o proteins, inhibiting adenylate cyclase activity and reducing cAMP levels, which influences neurotransmitter release and neuronal excitability. NPFFR2 is involved in nociceptive processing, functioning as an anti-opioid receptor to reduce the analgesic effects of opioids and contribute to opioid tolerance. Beyond pain regulation, NPFFR2 has been implicated in energy metabolism, feeding behavior, and stress responses. Dysregulation of NPFFR2 signaling is associated with chronic pain, opioid dependence, and metabolic disorders. Therapeutically, NPFFR2 modulation holds promise for improving opioid therapy by reducing tolerance and dependence while offering potential targets for metabolic and pain-related disorders.
