IL1R2 functions as a decoy receptor for interleukin-1, competitively binding to IL-1α, IL-1β, and the IL-1 receptor accessory protein (IL1RAP) without initiating downstream signaling IL1R2 lacks an active intracellular signaling domain, thereby preventing activation of NF-κB and other pro-inflammatory pathways IL1R2 is expressed on the surface of monocytes, macrophages, and B cells, and it is also secreted in a soluble form (sIL1R2), which further neutralizes IL-1 activity in the extracellular environment By acting as a negative regulator of IL-1 signaling, IL1R2 plays an anti-inflammatory role and contributes to the resolution of immune responses IL1R2 expression is upregulated by anti-inflammatory cytokines such as IL-4 and IL-10, highlighting its role in immune regulation Dysregulation of IL1R2 is associated with inflammatory diseases, including sepsis, arthritis, and chronic autoimmune disorders, where its expression may serve as a biomarker for disease activity IL1R2’s ability to limit IL-1 activity makes it a critical target for therapeutic strategies aimed at reducing excessive inflammation and immune activation
